Youth Sign-Up Student/Parent Registration Form

We want to make Sharehouse Youth Café & Community the best possible environment for learning, having fun and building authentic relationships. Sharehouse is a community so we expect students, volunteers and staff to adhere to our center expectations.

Respect is our biggest value at Sharehouse. Students should respect themselves, each other and the building. We want to spend as much time with your student as we can, not cleaning up after them.

Our goal is to empower students to reach their potential. We succeed in this only when we are able to build a relationship. For this reason, we expect all students to engage with peers, volunteers, and staff and to participate in our in-house programs.

In consideration for the privilege of allowing my child to participate in Sharehouse activities, I agree to release and hold harmless the Sharehouse Foundation, aka Sharehouse Youth Cafe & Community, its officers and agents, from any liability to or responsibility for bodily injury, damage or illness to the above-identified child while participating in any youth activity which may be directly or indirectly sponsored by the Sharehouse Foundation. Further, I agree to indemnify and hold harmless the Sharehouse Foundation, its officers and agents with respect to any claim asserted by or on behalf of my child as a result of bodily injury, illness, or damage.

Smoking, drugs, alcohol, or weapons are not allowed in or around the Sharehouse center, (this includes inside and outside areas of the facility). If staff or volunteers suspect that a student is under the influence of drugs or alcohol or possesses a weapon, the police will be called immediately. While at Sharehouse, you must be inside the building, not loitering outside.

Computers are available for homework first, then recreation. Those with homework always have first priority on the computers. Computers may not be used for chat rooms, porn, or any other inappropriate activity.
